<title><![CDATA[Day 64 (2008-09-02): The importance of self-care and full-body dentistry]]></title>

<content:encoded><![CDATA[<p>I called the dentist today to make an appointment and they had a cancellation for this afternoon, so I seized the opportunity. It fell within an <a href="http://www.turnontolife.com/integrity.html" target="_blank">Integrity Day</a>, which led to two important experiences for me.</p>
<p>First, the power of making a choice. Taking the afternoon appointment meant deviating from the plan I'd made for today. But I was aware of an inertia in me that was resisting, and I was also aware of a powerful place inside me telling me to act on what I knew was important. Making that choice decisively and without second-guessing it came from that deep peaceful place inside me and it led to a felt experience of peacefulness in that choice.</p>
<p>Second, having the dental appointment fall within the context of an Integrity Day helped me see that it wasn't a distraction, but an important part of my life as a whole. It's an act of self-care that could have been quite easily dismissed as an annoyance. Integrity Day is typically high-value time for me, so I got to experience self-care as high value.</p>
<p>Two other interesting notes:</p>
<ol>
<li>When they told me about the cancellation, they said, "Are you available at 2:30?" and I almost cracked up. That's one of my favorite (and dorkiest) jokes: What time do you go to the dentist? Tooth-hurty. Awesome coincidence.</li>
<li>It had been a while since I'd had a dental cleaning so there was lots of painful scraping. This gave me a chance to practice full-body awareness. When my focus was almost exclusively on my mouth, the experience was excruciating -- the pain, the scraping sounds. But when I expanded my focus to include my whole body, it wasn't that bad. Sure, there was still pain in my mouth, but my hands were fine and peaceful, my feet and legs were fine and peaceful, etc. The pain was a tiny part of my experience.</li>

While there is no question that mercury vapor is toxic, its release from amalgam fillings is very slow, and <strong>I'm not aware of any studies here or abroad showing that it presents a health risk.</strong> Most release occurs when fillings are put in, making it more hazardous for the dentists who work with amalgam regularly than for the patients whose teeth are being filled. <title><![CDATA[The Best Dentistry Is the Least Dentistry]]></title>

<content:encoded><![CDATA[<p>While a good number of people dread dental work, most of us tend to think of it as no big deal - not when compared to non-dental surgical procedures. A lot of this is probably due to the fact that despite the increasing amount or research confirming the link between oral and systemic health, we just don't think of the mouth as being connected to the rest of the body. The typical GP doesn't look at your mouth when you go in for a visit, and the typical dentist doesn't look at anything but your mouth. If your health care providers don't see the connection, why should you?</p>
<p>Also, with most dental work, you're not in much - if any - pain afterwards. You're more apt to feel just sort of wiped out. But this is one of the signs that the mouth has undergone significant trauma from the dental drill, syringes and other tools - and that your body is recovering.</p>
<p>In some cases, there might well be pain - something Dr. Verigin discusses at length in the most recent issue of <em><a href="http://www.biologicaldentalhealth.com/current_newsletter.shtml" target="_self">Biosis</a></em>. Repeated oral trauma and stress just from dental care can sometimes kick off a cascade of systemic effects. Left untreated - or inappropriately treated - this can lead to dysfunction and illness that eventually affects the whole body. We have seen it time and again from new clients who seek relief from long-standing oral and systemic health problems.</p>
<p>Because of this, we have always recommended taking the most conservative approach possible to dental work. As we say time and again, the best dental work is the least dental work. The best teeth are your own, and they can be beautiful without subjecting them to bleaching, veneers and other sources of dental stress and trauma.</p>
<p>The flipside to this approach is illustrated in <a href="http://www.mailonsunday.co.uk/health/article-1046622/Porcelain-veneers-ruin-teeth-smile--life.html" target="_blank">this recent essay from the <em>Mail on Sunday</em></a>. Rather casually seeking a quick fix for a Hollywood smile - porcelain veneers - the author wound up with a great deal of pain  and malocclusion (the teeth not coming together properly). This state of affairs is one we all too often see lead to fatigue, mood and emotional problems, and pain throughout the head, face, neck and back - problems that the author herself describes experiencing. Notably, this is all in addition to the initial trauma experienced in the teeth when they were ground down for placement of the veneers in the first place.</p>
<p>It's definitely a cautionary tale...</p>

<p>As Executive Vice President and Chief Medical Officer of Erickson Retirement Communities, Gary developed and managed the largest group practice (Erickson Health) of geriatrics providers in America. Erickson Health is recognized for providing care that is Accessible, Appropriate and Affordable for seniors. Currently, Erickson Health is staffed by 55 physicians and 30 advance practice nurses providing over 50,000 primary care visits/year with a budget that exceeds $20 million a year.</p>
<p>He also developed and provided medical direction to all services of the Erickson Health system including but not limited to: skilled nursing care, assisted living, rehabilitation services (PT, OT, SLP), pharmacy services, mental health, home health, podiatry, dentistry, numerous medical specialties, emergency medical services, and health clubs. Virtually all medical needs short of tertiary acute care are available at a fully developed Erickson community.</p>
<p>Gary lead the installation of Centricity (GE Health Care), a primary care electronic health record system, in less than a year at 10 practices in 7 states including data entry of over 10,000 patients’ charts. To accomplish this, the team needed to train over 200 providers and staff members to use system. Erickson’s practices are now paperless including all medical documentation, as well as prescriptions and billing. I have long considered electronic medical records essential to improving the U.S. health care system forward.</p>
<p>Most importantly, Gary loves seniors. Currently, he continues to practice medicine at Levindale Hebrew Geriatric Center and Hospital. He is also a partner and investor in an early stage venture capital fund, Nobska Ventures. In addition, he recently joined the team at <a title="TR Mann Consulting" href="http://www.TRMann.com" target="_blank">TR Mann Consulting</a>.</p>

<content:encoded><![CDATA[<p>Laser dentistry has been around for some time.  Laser has been claimed to casue less bleeding during surgery, faster healing  and painless cavity removal.  Personally, the only laser I owned was a pointer.  You know, the red laser dot pointer that you can use during a presentation in a dim room.</p>
<p>Having a laser in one's office may convey the notion that the dental office is high tech and everything the dentist does is up to date.  That is true.  Laser is after all high tech equipment.  But the question one needs to ask is how much  laser dentistry is better than conventional dentistry.</p>
<p>Some people have said that  a laser machine in a dental office is nothing more than a glorified "Electro-surge," an electiric surgery blade.  Basically they both use energy generated either by laser of electricity to cut through tissues.  If you are a dentist, you could read the research articles about laser.  If you are not a dentist, then you may want to ask your dentist about what scientific researches have found out about laser in dentistry.  You may also want to know that a dental laser machine costs quite a bit of money.</p>

<content:encoded><![CDATA[<p>It's that time of the year again. AADSAS is all turned in, DAT's are all taken. Now is the time when most of the schools out there start contacting potential candidates for interviews. Here are a few things that helped me on my interviews:</p>
<p>1) Bring a <a href="http://www.google.com/search?q=padfolio&#38;ie=utf-8&#38;oe=utf-8&#38;aq=t&#38;rls=org.mozilla:en-US:official&#38;client=firefox-a" target="_blank">Padfolio</a> with you in order to take notes in during financial presentations and such. Don't forget to bring something to write with.</p>
<p>2) Prepare some questions to ask your interviewer about the school/the curriculum/extracurricular activities. Chances are the interviewers will not just sit there and grill you with question after question. Most of my interviews had moments where I was able to ask questions, which in turn led to lengthy discussions of various topics. It is good to ask questions, even if you already know the answer, for it shows that you are interested in the school. Also, it helps to pass the time, and makes the interview seem shorter.</p>
<p>3) Dress to kill. Seriously. Get a suit if you are a guy. A ladies suit if you are a girl. Do not show up in Slacks and a Sport Coat. You will stand out, but not in a good way.</p>
<p>4) Do not chew gum. Use breath mints if you have to, but make sure they are the kind that aren't chewable, like altoids. You might have to wait a while for your interview (it's not always first thing in the morning) so if you are chewing gum, you might just forget that you are chewing it and walk into the interview still chewing it without even realizing it. This is not showing the interviewers that you communicate well.</p>
<p>5) Get to know the other people there with you, for they might be your future classmates. Due to that possibility, do not piss them off either.</p>
<p>6) Most importantly, relax and try to seem like you are having a good time being there. All of the interviews that I went on weren't stressful at all.</p>

<p>Ever since the FDA's turnabout on the health risks of mercury fillings, we've seen a steady stream of articles on the topic, such as <a href="http://health.usnews.com/articles/health/living-well-usn/2008/08/20/are-your-dental-fillings-containing-mercury-safe.html" target="_blank">this one</a> posted this week at <em>US News and World Report</em>. Like so many other MSM articles, it cites a pair of studies published in 2006:</p>
<blockquote><p>A large 2006 study found no statistically significant differences in IQ, motor function, or memory in children who had numerous mercury-containing amalgams. It did find that kids with amalgams had significantly higher mercury levels in their urine, but researchers observed no problems in kidney function that could have resulted from mercury poisoning. Another study, published in February in the <em>Journal of the American Dental Association</em>, supports the 2006 findings. Its Portuguese authors found no significant differences in brain function between a group of children whose cavities had been treated with amalgams and a group treated with composite resin material.</p></blockquote>
<p>Note that the information about the second study is wrong. The study referenced was also published in <em>JAMA</em>, not <em>JADA</em>, and its authors included six US- and five Portugal-based researchers.</p>
<p>In any event, both studies have been cited time and time again, as if they were the final word on mercury safety. They are not.</p>
<p>Dr. Verigin commented on these studies in the May 2006 issue of <em>Biosis</em>:</p>
<blockquote><p>In both current mercury studies, the researchers annually plotted their subjects' urinary mercury levels. For the first two years, as expected, the results showed a steady increase in the mercury levels of the amalgam group children. Then the results turned strange. These elevated mercury levels actually dropped until they were almost the same as those of the mercury-free children.</p>
<p>How could this be?</p>
<p>Writing in <em>Clinical Preventive Dentistry</em>, Chew, Soh, Lee and Yeoh noted that each average sized 1 gram amalgam filling contains 500,000 micrograms of mercury - about 53% of the total volume, on average. Through research, they concluded that with such a filling emitting a toxic 5 micrograms of mercury per day - emissions generated by chewing and grinding - it would take 100,000 days for all the mercury to be vaporized. As they say, "Do the math!" (Answer: about 274 years!) And how long is the average lifespan of an amalgam filling according to insurance standards? About 10 years.</p>
<p>So why the drop in mercury levels? The kidneys are the primary route of excretion here. I surmise that they are taking a major hit, becoming less effective at removal. I would expect more mercury to be stored. After all, the urinary mercury levels were measured without the intake of chelating agents, thus making the data unreliable. Moreover, recent research shows that over 90% of excreted mercury goes through the fecal rather than urinary route.</p>
<p>Most likely, the children's basic regulative systems became less flexible, more rigid. The autoregulatory pathways - liver, kidneys, skin, mucous membranes - all take part in the excretion of any toxin, including mercury. However, the detoxification process depends on variables such as genetics, gender and nutritional status. Thus, some people are less able to excrete mercury than others, even if they live in the same city, region or country, as in the current studies. Each will have a different diet with vastly different antioxidant and mineral intakes, as well as many of the chelators of mercury found in high quality protein.</p>
<p>A better measure of the effects of mercury would involve the immune system - something that was not mentioned in either study. The literature is packed with research on this. Mercury and other heavy metals with negative magnetic charges are powerful immune system suppressors. Under such conditions, the friendly microbes in our tissues undergo drastic changes. The major scavenging cells and immune response recognition cells, such as macrophages, have a lessened ability to render antigens harmless.</p>
<p>But instead of looking at this, the researchers focused on IQ.</p>
<p>They also focused on otherwise healthy children, even as the Centers for Disease Control report that 1 in 6 American children have some form of neurological disorder such as autism. Children so affected do not effectively excrete mercury. To exclude them - the individuals most susceptible to mercury toxicity - from the study is yet another major failing.</p>
<p>That said, it is my conviction that neither study as designed should have been performed at all. If living beings needed to be tested, then the experiments should have been done on some other form of primate, not human children. To subject unsuspecting children and their parents to such experimentation is the very definition of cruelty.</p>
<p>I question the ethics of these studies. I wonder if these researchers even remember the humanitarian motto of our profession: Doctor, do no harm unto your patient.</p></blockquote>

<content:encoded><![CDATA[<p>Root canal treatment - it's no big drama or mystery and gets very bad press.  Basically the centre of your tooth is hollow and filled with blood vessels and nerves just like the rest of your body.  If bacteria get in here you get swelling like any other cut or infection, except the swelling strangles the blood supply leaving an infected chamber in the tooth.  Without a blood supply, your body can't heal this area, and the toxins spread out from the root into the surrounding bone eventually causing swelling, pain or damage to the root.</p>
<p>Root canal treatment involves cleaning away the bacteria and toxins - imagine flushing out termites from an 'infected' tree.  Then a filling material is placed into the hollow root chamber to prevent further bacteria getting back in.  Just as a dead tree doesn't fall out of the ground, a 'dead tooth' doesn't fall out either.</p>
<p>Here is a picture of an infected front tooth - the dark area around the root is the extent of the infection.  The white areas are all filling - basically this tooth has been filled to death from larger and larger fillings.</p>

<p style="text-align:left;">Most teeth need a crown to protect the seal and rebuild the tooth due to the damage from the original decay, large filling, or trauma.</p>
<p style="text-align:left;">Myth: pain - more people fall asleep during root treatment for me than any other procedure.  If the tooth is properly numb then it is a time consuming but uneventful procedure.  Some people may feel pain if the tooth is acutely inflammed and infected - and this may mean a temporary sedative dressing for a few weeks and treating when the infection has settled.  With patience and a gentle technique it is not a difficult procedure.</p>
<p style="text-align:left;">Myth - I had root treatment and the tooth was pulled out anyway. - some treatments do fail.  A large number fail from not sealing over with a crown - the filling compromise leaked or cracked.  Some teeth should be done by a specialist - an endodontist, if the roots appear really twisted or hard to clean thoroughly.</p>
<p style="text-align:left;">The most important thing to know is that root treatments become the only option a dentist has to save your tooth if you wait until you are getting pain before you come to the dentist.  Your choices in dentistry are:</p>
<p style="text-align:left;">1 - small cavity found during check-up means a small filling needed</p>
<p style="text-align:left;">2- leave the hole a while a large filling and risks that the tooth will need a crown or root canal treatment 5 or 10 years down the track</p>
<p style="text-align:left;">3- leave the hole till it starts getting sensitive means a large filling that may or may not work and probable root treatment between 1 week and a few years in the future</p>
<p style="text-align:left;">4 -leave until a throbbing toothache means root treatment now to save the tooth, and it may not go numb and could hurt more and need a dressing</p>
<p style="text-align:left;">5 - leave till the tooth rots further and pain goes - only alternative is extraction and risks of serious infection.  Although rare people still do get hospitalised and even die from dental infection.</p>
<p style="text-align:left;">It is an old but true saying - "<strong>A stich in time saves nine"</strong></p>

<content:encoded><![CDATA[<p>The "standard of care" is among the <strong>most pretentious and misused gimmicks in health care</strong>, and I would like to see those words eliminated entirely from the diction of my colleagues.</p>
<p>I ran a search for "standard of care definition." Let's see what <a href="http://www.medterms.com/script/main/art.asp?articlekey=33263" target="_blank">our highest ranking page</a> has to say about the standard of care.....</p>
<p><strong>Standard of care:</strong></p>
<p><strong>1.</strong> A diagnostic and treatment process that a clinician should follow for a certain type of patient, illness, or clinical circumstance. Adjuvant <a href="http://www.medterms.com/script/main/art.asp?articlekey=7778">chemotherapy</a> for <a href="http://www.medterms.com/script/main/art.asp?articlekey=406">lung cancer</a> is "a new standard of care, but not necessarily the only standard of care." (New England Journal of Medicine, 2004)</p>
<p><strong>2.</strong> In legal terms, the level at which the average, prudent provider in a given community would practice. It is how similarly qualified practitioners would have managed the patient's care under the same or similar circumstances. The medical malpractice plaintiff must establish the appropriate standard of care and demonstrate that the standard of care has been breached.</p>
<p>The <strong>standard of care is</strong> <strong>not a concrete concept with a singular interpretation</strong>, yet many people continue to employ it as though it were. It is a poor abstraction of the logic of average practitioners that can only actually be applied inside a courtroom. Outside the courthouse, it is nothing more than a legal bogeyman, and has absolutely no place dictating care.</p>
<p>Clinical decisions should rely on a knowledge of the literature, your materials, and a history of clinical experiences. Anyone who enacts real clinical judgment sees the standard of care for what it really is, a <strong>pathetic euphemism for a cookbook style approach to health care</strong> that is accepted by average doctors who lack the desire to achieve excellence in their field.</p>

<p>So, this morning, Blair was able to squeeze me in to get my teeth cleaned at the school. I have to admit that I was a little bit nervous, but for no good reason. Blair was awesome, and I'm now convinced that they ARE teaching him something!</p>
<p>For so long, this life that Blair had been living what with "school" and "clinic" had been a little bit surreal. I mean, how was I supposed to know that he was <em>really </em>in there practicing dentistry when I'd drop him off at the front door? Well, it turns out that it's for real, Blair is going to be a dentist, and is already quite good, if I do say so myself. Just check out my pearly whites that he cleaned so nicely! (ignore the baby fat...hopefully we'll have that taken care of here soon...)</p>

<h3><span style="color:#30ce6f;">ok.. gw tau ini akibat ga pernah makan pagi n tidur kemaleman, jadinya gw tambah bego.. but i knw God will help me =) (percaya Tuhan!!) hehe.. gw bakal berusaha berubah deh</span></h3>

(dengan suara agak keras) i cleaned it alredy" tapi dia tuh maksa2 terus, akhirnya gw masuk ke clinic, dan ternyata gw cuma disuruh ngebersihin darah (keciil banget) di waste receptacle d dental chair,, (well, emang harus dibersiin sih, tapi gw lg ngantuukkk!!! ahh) abis itu, pas gw bersihin, masa dia bilang itu kurang bersih, gw gataw ya dia ngerjain gw ato apa, tapi gw kesseeeeelll banget, kenapa sih tuh orang ga bisa diem sekali aja kl ktm gw?!</span></h3>
<h3><span style="color:#30ce6f;">akhirnya gw diem aja, gw ambil barang2 gw yg laen, gw buang gloves, mask ama bib ke tempat sampah terus gw cuci barang2 gw n gw keluar dr clinic,</span></h3>

(acrylic bur itu kecil banget dan harganya 1 biji 900 peso, gw punya pernah hilang dan sekarang gataw dmana) terus gw bilang "hello?!" tapi gw check juga (barang2 gw uda di sterilizer) dan acrylic bur itu gak ada.</span></h3>
<h3><span style="color:#30ce6f;">ternyata.. acrylic bur nya punya grace dan grace kasi pinjem ke reuel tapi jin ngambil itu dari reuel karna mo dikasi ke chris, trs J tinggalin acrylic bur (AB) nya di sink tempat barang2 gw yg belom dicuci tadi..</span></h3>

<h3><span style="color:#30ce6f;">abis itu, jam 2-an gitu gw tidur2an d dental chair rum.5, taunya.. ketiduran, ampe jam 3.30.. yawda bangun2 gw ke library dent, ngobrol sama doc.carpio (guru favorite gw!!) katanya gw ga jadi exam, tapi assignmentnya buanyak! terus.. gw nanya ke dia "why do we have to do thesis???" then she said "becos u want to graduate" then i said "why do we have to defend r thesis?" she answered "becos you want to graduate" OMG!!! gw gataw mo bilang apa.. apicoectomy datanglaaaahhh!!! anterior bridge, datanglaaaahhh!!!</span></h3>
<h3><span style="color:#30ce6f;">berhubung flashdisk gw patah, gw dikasi flashdisk ama doc.mabunga (thank God) berkat gw uda banyak banget di dent,, i got everything i need at the right time, Tuhan itu emang baik banget,, jadi gw ga bakalan takut lagi.. gw tau kok gw pasti tamat, tapi gataw kapan =)</span></h3>

Today is the day that the Placing Results of the Student Selection Exam were out and guess what, it]]></description>

<p>Today is the day that the Placing Results of the Student Selection Exam were out and guess what, it started with a joke but seems like I will become a dentist. I made it into Erciyes University Dentistry Faculty.</p>
<p>It's common knowledge that studying dentistry is way the most pocket-burning one. I'll see about that by first-hand experiences.</p>
<p>As far as I heard from an older bro who studies dentistry, you always keep asking yourself during your the very first months of the first year "why dentistry? why not a damn normal engineering but dentistry?", but after that you start being fellow-fellow with patients, eat your toast in cafeteria with them, they tell you about their family problems, you give them advices, blah blah ..</p>
<p>The lessons start by a hard Latin in them and without even realizing what's going on, you go into the preclinic laboratory to make teeth from soap. Your eyes do get used to soap time after time although you'll have to walk around with red eyes like you just cried, but some time later you'll also walk around like you're the best sculptor in the country. "Teeth?! Come on! I can make a bicycle out of soap!" .. like this.</p>
<p>Second year, people will stay a meter away from you since you'll smell like you have bathed in an acryl cauldron and you'll also have to make your homeworks windows wide-open in the middle of a cold winter putting cloth on cloth on you, because the other people in the house won't be so happy with the smell (they'll be studying clean engineerings, ofcourse they'd be annoyed) .. You'll write lyrics comprising from "I hate it, I hate it, dentistry? Why is it? Why is it? I hate it, la la la la laaa, I hate this .." while you'll be making your homework for tomorrow in the middle of the night at 3 a.m. And it will depend on the mood your professor would be in, that either he'll accept your work or simply say a "do it again" .. You'll start singing that song again.</p>
<p>Third year would be like the second one. You'll just develop this song of yours into a better one with a better rythim.</p>
<p>You'll spend your time running around trying to find a patient to use not to fail the exam at your fourth year. By the phone you'll beg them to come when you're about to have an important exam and your patient is having tea with her middle-aged friends at an at-home day.</p>
<p>Fifth year is the last one. After graduation you stay at home in front of tv watching it like an idiot since you're in an empitiness, not knowing what to do but it won't take so long and you'll find a job somehow inshaallah.</p>

<p>Ok, finally pull my ass together to do a proper post. So the story was that I had my half BDS last Saturday. More bout that later. It is my blog afterall, so it cannot be me if there ain't any complain bout my studies lolol. I had my first complex pros session that day and did I tell you it was sibeh damn hard? Omg. I thought I did a very good job on the labial surface. Considering it's my first one la. But when it comes to palatal. OMFG. So freaking ugly! I ruined the entire prep T-T. From the front it still looks good. But once the mirror comes along....zzz. This is what we call 'yuen tai yat zi fa, kan tai *** *** ** **' you fill in the blanks la. I cannot bring myself to speak such vulgarities hahaha. It's too extreme for my standard. Plus I hate scolding other people's mother. They're like innocent ok, why drag them into the context. Even though it's a dead extracted tooth we're talking about here...</p>
<p>Anyway. Then I finally started my clinical radiography sessions. It was kind of boring. OPG. PAs. OPG. OPG. Another PA. OPG. Zzzz. Until this girl comes along. For ortho consultation. So we get to do the 'ALL' of radiographs I think? PAs, BWs, OPG, Lat ceph and hand wrist. So cool! Anyway, one last session to go on Friday. Here's to hoping it all goes well....</p>
<p>So back to the Half BDS. It was to mark the fact that BDS Class of 2010 has reached halfway mark of the course. Two and a half years down. Woo hooooooo~~~~ Another two and a half more to go. Sighs. But at least we got by it. So yea. The night was very fun! Was worth the money definitely. It was held in Ayers House, right next to Wendy's apartment lol. Easy for her neh.</p>
<p>So now to the pictures.... There's like thousands of them haha, so please be patient. Will need to do them slowly, especially when I am pinching other people's internet haha. Here we go for the arbitary and edible things.</p>

<p>The food was soooooo yummy. Btw,I didn't eat all that la. I shared with Yinan so that we can tried all the food in the menu. It didn't really matter in the end, cos they're all good ^^. Then there's free flow of drinks. Can't get better than that lol. I tried to drink the money back hahaha. But nola, I do care about my liver thank you very much. Plus don't want to get alcohol intoxication like some girl that night. Or even puke 3 times like how yet another girl did. Or sleep in the bar like cough cough* asdf *cough cough. It's not worth it la. Supposed to enjoy it, not ruin it. Plus people who get drunk always ruin it for others. And I so don't wanna be one of those people, very selfish lor. No offence to you asdf.</p>
